Introduction to AI diagnostics
Artificial intelligence is transforming dentistry in ways that were unimaginable until recently. The most visible change is in diagnostics — specifically, in the analysis of panoramic X-rays and intraoral photography.
While an experienced dentist spots caries or periapical lesions, AI here acts as a consistent “second pair of eyes” — it doesn't tire and applies the same criteria every time, which is especially helpful for early, easily missed changes.
What does AI actually do with X-rays?
Modern AI models for dentistry use convolutional neural networks (CNNs) trained on millions of annotated images. The system learns to recognize:
- Early caries — before it becomes visible to the naked eye
- Periapical lesions — inflammatory changes at the root tip
- Bone resorption — early stage of periodontal disease
- Impacted teeth — especially wisdom teeth in unfavorable positions
- Calcifications — which may indicate systemic diseases
Tools worth a look
Pearl — Second Opinion
Real-time detectionAn FDA-cleared system that flags suspected findings live as you view the image — caries, periapical changes, impacted teeth — on periapical, bitewing and panoramic radiographs. Built for chairside conversations with the patient.
Apteryx Imaging AI
Caries detectionA cloud imaging system (XVWeb) with AI support that highlights suspected caries areas on the radiograph. Integrates into the practice's digital workflow.
Diagnocat
CBCTOne of the most sophisticated systems — analyzes CBCT scans, not just panoramic. Price is higher, but the value for oral surgeons is enormous.
Limitations of AI diagnostics
It's important to understand that AI is not a replacement for the dentist — it's an assistant. The system may miss contextual information (patient symptoms, medical history, clinical examination) that are crucial for an accurate diagnosis.
